Independent Contractor Authorization (ICA) Form Training

The Office of Disbursement Services, in collaboration with the Payroll Office and the Grants Management Support Center, is introducing the Independent Contractor Authorization (ICA) Form, which will be used to support the appropriate classification and processing of independent contractor requests in compliance with institutional and state requirements. This form will replace the existing Authorization for Personal Services (APS) Form. The APS Form will continue to be accepted until September 1, 2026; however, for improved efficiency and processing timeliness, departments are strongly encouraged to begin using the ICA Form moving forward.
